Proximal Hamstring Avulsion

Complete or partial tearing of hamstring muscles from their origin at the ischial tuberosity, typically occurring during explosive athletic movements.

Proximal hamstring avulsion represents a serious injury involving the detachment of one or more hamstring muscles (biceps femoris, semitendinosus, or semimembranosus) from their origin at the ischial tuberosity. This injury predominantly affects sprinters, soccer players, and athletes involved in activities requiring sudden acceleration or extreme hip flexion with knee extension. The mechanism typically involves eccentric contraction during high-speed running or water skiing. Unlike muscle belly strains, proximal avulsions often require surgical intervention, particularly when there is significant retraction of the muscle belly. Clinical presentation includes severe posterior thigh pain, palpable defect, and characteristic 'popping' sensation at injury onset.
