Scaphoid nonunion occurs when a scaphoid fracture fails to achieve bony healing within the expected timeframe, typically defined as absence of radiographic union after 3-6 months of appropriate treatment. This complication is relatively common due to the scaphoid's poor blood supply, particularly in the proximal pole. Risk factors include delayed diagnosis, inadequate initial immobilization, proximal pole location, and patient noncompliance. Untreated scaphoid nonunion can progress to SNAC (scaphoid nonunion advanced collapse) wrist arthritis and requires surgical intervention.
