Rotational malalignment refers to the improper rotational positioning of bone fragments following fracture treatment, where the normal anatomical rotation of the bone is not restored or maintained. This complication can occur in long bone fractures, particularly in the femur, tibia, and humerus, and may result from inadequate reduction, loss of reduction, or healing in an improper position. Rotational malalignment can cause significant functional problems including altered gait mechanics, joint dysfunction, and cosmetic deformity.
