Talar tilt is measured on a stress inversion radiograph as the angle between the tibial plafond and the superior surface of the talar dome. It quantifies the degree of lateral gapping within the ankle mortise when the ankle is stressed into inversion, indicating the integrity of the calcaneofibular and anterior talofibular ligaments. A talar tilt of more than 5–10° (depending on the reference standard) is generally considered evidence of ligamentous laxity. In orthotic practice, significant talar tilt may indicate the need for lateral ankle stabilisation via a brace, ankle-foot orthosis, or rigid footwear.
