Medial Wedge

An orthotic insert or outsole modification that is thicker on the medial (inner) side, applying a varus force to the foot or limb segment.

Full Definition

A medial wedge is a ramp-shaped piece of material — built into an insole, midsole, or outsole — that elevates the medial border of the foot relative to the lateral border, thereby inducing subtalar supination and reducing excessive pronation. It is a common intervention in the conservative management of medial knee osteoarthritis, overpronation, and tibialis posterior tendon dysfunction. Editors must distinguish it from a 'lateral wedge,' which has the opposite orientation and produces the opposite biomechanical effect. In prescriptions and billing documents, the specific degree of wedging (e.g., 4° medial wedge) is clinically significant and must be transcribed accurately.

Usage

Usage note: Always specify side (medial vs. lateral) and location (forefoot vs. rearfoot vs. full-length) in clinical documents. A medial forefoot wedge and a medial rearfoot wedge have different biomechanical effects and must not be conflated.
