A total contact cast (TCC) is regarded as the gold standard offloading intervention for neuropathic plantar ulcers associated with diabetic peripheral neuropathy. Unlike a conventional plaster cast, a TCC is applied with minimal padding so the casting material contacts the full plantar surface, diffusing weight-bearing forces. Because it is non-removable, patient compliance is inherent. In clinical documentation, editors must note that 'total contact cast' is distinct from a 'removable cast walker' or 'CAM boot,' even when the walker is rendered non-removable by strapping.
