Full Definition

High-frequency oscillatory ventilation (HFOV) is a specialized form of mechanical ventilation that uses very high respiratory rates (typically 180-900 breaths per minute) with extremely small tidal volumes, often less than anatomical dead space. In pediatric critical care, HFOV is primarily used as rescue therapy for severe hypoxemic respiratory failure when conventional ventilation fails or as a lung-protective strategy in conditions such as severe ARDS. The technique maintains lung recruitment through continuous positive airway pressure while oscillations create gas exchange through mechanisms beyond simple bulk flow. HFOV can be particularly beneficial in pediatric patients due to their smaller airways and different respiratory mechanics compared to adults.
