Hemispherotomy is a neurosurgical technique that achieves functional disconnection of one cerebral hemisphere without removing large amounts of brain tissue, unlike traditional hemispherectomy. The procedure involves strategic disconnection of key white matter tracts and structures while leaving most of the hemisphere in place. This approach reduces complications such as hydrocephalus and superficial cerebral hemosiderosis while maintaining seizure control efficacy. It is typically performed for medically refractory epilepsy arising from one hemisphere in cases of extensive unilateral brain damage.
