Aniseikonia is a binocular vision disorder characterized by a significant difference in the perceived size or shape of images between the two eyes, typically greater than 3-5%. This condition can result from differences in retinal image magnification due to anisometropia, surgical factors such as IOL implantation, or retinal pathology. In pediatric patients, aniseikonia may develop following unilateral cataract surgery with IOL implantation or significant anisometropic correction. The condition can cause diplopia, eye strain, and suppression of the affected eye, potentially leading to amblyopia. Specialized testing with aniseikonia meters or computer-based assessments may be necessary for diagnosis and monitoring.
