Juvenile idiopathic scoliosis (JIS) refers to spinal curvature of unknown etiology that develops between ages 4 and 10 years, representing approximately 10-15% of all idiopathic scoliosis cases. Unlike adolescent idiopathic scoliosis, JIS carries a significantly higher risk of progression, with curves often advancing relentlessly throughout the remaining growth period. The increased progression risk is attributed to the substantial remaining spinal growth and the longer duration until skeletal maturity. Early-onset scoliosis, including JIS, can also impact pulmonary development, particularly when thoracic curves develop before age 10. Treatment strategies include observation for small curves, bracing for moderate curves (typically 25-45 degrees), and surgical intervention for larger or rapidly progressing curves. Growing rod constructs or guided growth techniques may be employed to control progression while preserving spinal and chest wall growth potential.
