Full Definition

The lateral pillar classification, developed by Herring, is a prognostic classification system for Legg-Calvé-Perthes disease that evaluates the extent of involvement of the lateral portion of the femoral head during the fragmentation stage. The system divides cases into three groups: Group A (no lateral pillar involvement), Group B (partial lateral pillar involvement with maintained height), and Group C (significant lateral pillar involvement with collapse). A Border B/C group was later added for cases with characteristics of both B and C. This classification helps predict prognosis and guide treatment decisions, with Group A having excellent prognosis, Group B having variable outcomes depending on age, and Group C typically having poor prognosis without intervention.
