Limb-length inequality (LLI) refers to a measurable difference in length between paired extremities, most commonly affecting the lower limbs. In pediatric patients, this can result from congenital conditions, growth plate injuries, infections, tumors, or neuromuscular disorders. The discrepancy may be classified as functional (apparent shortening due to contractures or deformity) or structural (true bone length difference). Clinical significance depends on the magnitude of discrepancy, with differences greater than 2-3 cm typically causing noticeable gait abnormalities. Management options include observation, shoe lifts, epiphysiodesis of the longer limb, or limb lengthening procedures, with treatment decisions based on predicted final discrepancy, patient age, and functional impact.
