The Risser sign is a radiographic marker of skeletal maturity that assesses the degree of ossification of the iliac crest apophysis on anteroposterior pelvis or spine radiographs. The sign is graded from 0 to 5: Grade 0 indicates no ossification, Grades 1-4 represent progressive ossification from lateral to medial (25%, 50%, 75%, and 100% respectively), and Grade 5 indicates complete fusion of the apophysis to the iliac crest. This grading system is particularly valuable in pediatric orthopedics for assessing remaining spinal growth potential in patients with scoliosis, helping guide treatment decisions regarding bracing versus surgical intervention.
