A thoracolumbosacral orthosis (TLSO) is a custom-fitted rigid spinal brace that extends from the upper thoracic region to the pelvis, designed to control the progression of spinal deformities such as scoliosis and kyphosis in skeletally immature patients. The brace works by applying corrective forces through strategically placed pads while allowing for continued growth. Modern TLSO designs focus on patient comfort and compliance while maintaining corrective efficacy. The brace is typically worn 18-23 hours per day until skeletal maturity is reached. Success depends on curve type, magnitude, patient compliance, and remaining growth potential.
