ALARA (As Low As Reasonably Achievable) is a fundamental radiation protection principle that is especially important in pediatric radiology. Children are more sensitive to radiation than adults and have longer life expectancies, increasing their lifetime cancer risk from medical radiation exposure. ALARA principles in pediatric imaging include using the lowest radiation dose that produces diagnostically adequate images, employing alternative imaging modalities when appropriate (ultrasound, MRI), and ensuring proper collimation, shielding, and technique factors. Many pediatric radiology departments follow the Image Gently campaign guidelines.
