Articular cartilage thinning refers to the progressive loss of the smooth cartilage layer that covers joint surfaces, resulting from chronic inflammatory processes in pediatric rheumatic diseases. Unlike bone erosions, cartilage thinning may be partially reversible with effective treatment, particularly in growing children. This finding is best detected using MRI or ultrasound and represents an intermediate stage between active synovitis and irreversible joint damage. Monitoring cartilage thickness helps assess treatment effectiveness and guides decisions about therapy intensification. Early detection and treatment of cartilage thinning may prevent progression to permanent joint deformity.
