Full Definition
Minimal change nephritis is a pattern of lupus nephritis characterized by minimal histologic abnormalities on light microscopy but with characteristic changes visible only on electron microscopy, including effacement of podocyte foot processes. In children with systemic lupus erythematosus, this form of nephritis may present with significant proteinuria, sometimes reaching nephrotic range, despite relatively preserved kidney function. Unlike primary minimal change disease, lupus-associated minimal change nephritis may progress to more severe forms of nephritis and requires immunosuppressive treatment rather than corticosteroids alone. Regular monitoring and repeat biopsies may be necessary as the histologic pattern can evolve over time.
