Testicular torsion is a urological emergency characterized by rotation of the testis and spermatic cord, resulting in compromised blood flow to the affected testis. It presents with acute onset of severe scrotal pain, nausea, and vomiting. Time to treatment is critical, as testicular viability decreases significantly after 6 hours of ischemia. In pediatric patients, torsion has a bimodal distribution with peaks in neonates and adolescents. Immediate surgical detorsion and bilateral orchidopexy (fixation) are required to salvage the affected testis and prevent contralateral torsion.
