Full Definition

Periventricular echogenicity refers to areas of increased echogenicity (brightness) visualized on cranial ultrasound imaging in the white matter surrounding the lateral ventricles of the brain. This finding is particularly significant in premature infants and represents a spectrum of white matter injury ranging from transient changes due to normal maturation to more serious conditions such as periventricular leukomalacia or intraventricular hemorrhage with periventricular extension. The echogenicity is graded based on intensity relative to the choroid plexus, with persistent echogenicity beyond two weeks of age being more concerning for permanent injury. Serial ultrasounds are typically performed to monitor progression and guide neurodevelopmental follow-up care.
