Open flap debridement (OFD) is a periodontal surgical technique performed when non-surgical therapy has failed to adequately resolve deep pockets or complex bone defects. The mucoperiosteal flap is reflected to expose the root surfaces and supporting bone, enabling more thorough removal of calculus, biofilm, and granulation tissue under direct vision. OFD is distinguished from more advanced resective or regenerative procedures by the absence of osseous modification or grafting.
