Resistance mutations are genetic changes that arise during cancer treatment and confer survival advantage to cancer cells in the presence of therapeutic agents. These mutations can be primary (present before treatment) or acquired (developing during therapy) and typically affect drug targets, downstream signaling pathways, or alternative survival mechanisms. Common examples include EGFR T790M mutation causing resistance to first-generation EGFR inhibitors and various ABL kinase domain mutations in BCR-ABL1-positive leukemias. Understanding resistance mechanisms enables development of next-generation therapies and combination strategies.
