Engagement occurs when the presenting part of the fetus (usually the head in vertex presentations) descends to the level of the maternal ischial spines, corresponding to station 0. Engagement may occur weeks before labor in primigravidas or not until active labor in multiparous women. Assessment of engagement is part of routine obstetric examination and Leopold's maneuvers.
