Exercise stress testing (EST) is a non-invasive diagnostic procedure that monitors heart rhythm, blood pressure, and symptoms during progressive physical exertion, typically on a treadmill or stationary bicycle. The test can detect exercise-induced ischemia, assess functional capacity, evaluate blood pressure response to exercise, and provide prognostic information. Standard protocols include the Bruce protocol for treadmill testing and staged bicycle ergometry. Key endpoints include achievement of target heart rate (85% of age-predicted maximum), development of chest pain, significant ECG changes, or hemodynamic instability. In preventive cardiology, EST helps risk-stratify asymptomatic patients and guide exercise prescription. Normal exercise capacity and absence of ischemic changes are associated with excellent prognosis.
