PCSK9 inhibitors are monoclonal antibodies that block the proprotein convertase subtilisin/kexin type 9 enzyme, which normally degrades LDL receptors in hepatocytes. By inhibiting PCSK9, these medications increase LDL receptor availability, dramatically enhancing cholesterol uptake by the liver and reducing circulating LDL cholesterol by 50-70%. Currently available agents include evolocumab and alirocumab, which are administered as subcutaneous injections every two weeks or monthly.
