Syndrome characterized by subclinical psychotic symptoms that may precede the onset of full psychotic disorder.

Full Definition

A clinical condition defined by the presence of attenuated positive psychotic symptoms, brief limited intermittent psychotic symptoms, or genetic risk combined with functional decline. Individuals in clinical high-risk states experience subclinical psychotic symptoms that are not severe enough to meet criteria for a psychotic disorder but represent a significant risk factor for developing schizophrenia or other psychotic disorders. This concept is central to early intervention strategies and preventive psychiatry, as it identifies a window of opportunity for intervention before the onset of full psychotic illness. Clinical high-risk states are assessed using specialized instruments and criteria, and their identification has important implications for treatment planning and prognosis.

Usage note: Often abbreviated as CHR, but spell out on first use. Distinct from 'prodrome' which refers to retrospectively identified early symptoms.
