Full Definition
A clinical intervention requiring a staff member to maintain constant visual contact with a patient who poses immediate risk to self or others. The observer remains within arm's length and documents the patient's behavior at regular intervals. This level of supervision is typically ordered by a physician and requires specific documentation justifying the need. The observation may be upgraded to arm's length supervision if the risk increases.
