A treatment plan is a detailed, individualized document that serves as a roadmap for a patient's psychiatric care, developed collaboratively by the treatment team and patient. It includes assessment findings, diagnostic formulation, specific therapeutic goals, evidence-based interventions, assigned responsibilities, target dates, and criteria for measuring progress. Treatment plans must be regularly reviewed and updated to reflect changing needs and treatment response. They serve as communication tools between providers, documentation for insurance coverage, and legal records of planned care. Effective treatment plans are specific, measurable, achievable, relevant, and time-bound (SMART goals).
