The field-in-field technique is an advanced beam shaping method in external beam radiation therapy where multiple open fields with progressively smaller jaw settings are superimposed to achieve improved dose uniformity. This technique eliminates the need for physical wedges by creating a dose gradient through the strategic placement of subfields within the primary treatment field. The method is particularly effective for treating areas with irregular contours, such as the chest wall or head and neck regions, where standard rectangular fields would create unacceptable dose heterogeneity. Modern treatment planning systems can optimize field-in-field arrangements automatically, calculating the appropriate monitor units for each subfield to achieve the desired dose distribution.

Usage note: Often abbreviated as FIF in clinical documentation. Distinguish from sliding window IMRT, which uses moving leaves rather than multiple static fields.
