The inferior gluteal artery perforator (IGAP) flap is a microsurgical breast reconstruction technique that uses tissue from the lower portion of the buttock, supplied by the inferior gluteal artery. This flap preserves the gluteal muscle while providing adequate tissue volume for breast reconstruction. It is often chosen when DIEP or TRAM flaps are not suitable due to previous abdominal surgery or insufficient abdominal tissue. The procedure requires microsurgical anastomosis to the recipient vessels in the chest.
