PEEP titration is the process of systematically adjusting positive end-expiratory pressure levels to achieve optimal balance between improved oxygenation and minimized risk of ventilator-induced lung injury. Respiratory therapists use various approaches including PEEP/FiO2 tables, pressure-volume curves, or incremental PEEP trials while monitoring parameters such as oxygenation, hemodynamics, and lung compliance. The goal is to find the optimal PEEP level that recruits collapsed alveoli and improves ventilation-perfusion matching without causing overdistention or hemodynamic compromise. This process requires careful monitoring and frequent reassessment as patient conditions change.
