SIMV is a partial ventilatory support mode that combines controlled mandatory ventilation with spontaneous breathing capabilities. The ventilator delivers a preset number of mandatory breaths per minute, synchronized with the patient's inspiratory efforts when possible. Between mandatory breaths, patients can take spontaneous breaths at their own tidal volume and rate. This mode is commonly used during weaning from mechanical ventilation as it allows gradual reduction of ventilatory support while maintaining backup ventilation. Modern SIMV often includes pressure support for spontaneous breaths.
