Full Definition
An extensive surgical procedure involving en bloc removal of the entire lung, visceral and parietal pleura, ipsilateral pericardium, and hemidiaphragm. This radical operation is primarily performed for malignant pleural mesothelioma in carefully selected patients with good performance status and adequate cardiopulmonary reserve. The procedure requires reconstruction of the pericardium and diaphragm using synthetic materials or biological grafts. Extrapleural pneumonectomy carries significant morbidity and mortality risks and is only performed at specialized centers. Patient selection criteria include absence of mediastinal nodal involvement, good pulmonary function, and absence of significant comorbidities.
