Full Definition

The lepidic growth pattern is characterized by neoplastic cells spreading along the surface of alveolar septa in a non-invasive manner, preserving the underlying lung architecture. This growth pattern is the defining feature of adenocarcinoma in situ and is a component of minimally invasive adenocarcinoma. When lepidic growth predominates in invasive adenocarcinomas, it defines the lepidic-predominant subtype, which generally has a more favorable prognosis. Recognition of lepidic patterns is crucial for accurate histological classification and staging, as purely lepidic growth (without invasion) indicates earlier-stage disease with excellent prognosis when completely resected.
