Grade V splenic injury represents the highest level of splenic trauma in the American Association for the Surgery of Trauma (AAST) organ injury scale. This classification indicates complete splenic disruption or devascularization of the spleen, typically requiring immediate surgical intervention. Grade V injuries involve either shattered spleen, devascularization, or hilar injury with associated parenchymal destruction. These injuries carry high morbidity and mortality and almost invariably require splenectomy or complex surgical repair.
