Hemodynamic reserve represents the cardiovascular system's ability to augment blood flow beyond resting levels in response to physiological stress, pharmacological vasodilation, or increased metabolic demands. Assessment of hemodynamic reserve provides critical information about the functional significance of vascular lesions and the adequacy of collateral circulation. Testing methods include exercise stress, pharmacological vasodilators, and CO2 challenge protocols combined with various imaging modalities. Reduced hemodynamic reserve indicates compromised vascular function and increased risk of ischemic events. This parameter is particularly valuable in cerebrovascular disease assessment, where cerebral vascular reserve testing guides intervention decisions and risk stratification.
