Wire entrapment is a procedural complication that occurs when a guidewire becomes trapped or caught within the struts of a deployed stent, between device components, or within tortuous anatomy, preventing wire removal or manipulation. This complication can occur during various endovascular procedures but is most commonly seen in complex coronary or peripheral interventions involving multiple devices, bifurcation stenting, or when using multiple guidewires simultaneously. Wire entrapment can lead to vessel damage during attempted forceful removal, device dislodgment, or requirement for additional interventional procedures to free the wire. Prevention strategies include careful wire management, appropriate device selection, and systematic approach to multi-wire procedures. Treatment options range from gentle manipulation techniques to deployment of additional stents to trap the wire against the vessel wall, or in severe cases, surgical removal may be required.
