Full Definition
The gold standard conduit for infrainguinal arterial reconstruction, utilizing the patient's own greater saphenous vein as a bypass conduit. The vein may be used in situ (leaving it in its natural position with valve destruction) or as a reversed graft (removing and reversing orientation). Advantages include excellent biocompatibility, resistance to infection, and superior long-term patency compared to prosthetic grafts. Disadvantages include limited availability, potential harvest site morbidity, and size mismatch issues. The saphenous vein is preferred for distal reconstructions to below-knee vessels where prosthetic grafts have poor patency rates.
