Physiologic exhaustion describes a critical state where a patient's metabolic reserves are depleted and normal compensatory mechanisms have failed, making continued or prolonged surgical intervention life-threatening. This condition is characterized by the development of hypothermia, metabolic acidosis, and coagulopathy - collectively known as the 'lethal triad.' In acute care surgery, recognition of physiologic exhaustion is crucial for determining when to abort or abbreviate procedures in favor of damage control techniques. Signs include core temperature below 35°C, pH less than 7.2, base deficit greater than 15, and clinically evident coagulopathy. Recovery requires intensive care support, aggressive warming, correction of acidosis, and blood product resuscitation.
