Vasodilator stress testing is a pharmacologic method of cardiac stress testing used when patients cannot achieve adequate exercise levels or when combined with imaging modalities such as nuclear perfusion scanning or stress echocardiography. Common vasodilating agents include adenosine, dipyridamole, and regadenoson, which work by causing coronary vasodilation and creating a flow differential between normal and stenotic coronary arteries. In areas supplied by significantly stenosed vessels, the inability to increase flow appropriately creates relative hypoperfusion that can be detected by imaging. This form of stress testing is particularly useful in patients with physical limitations, orthopedic problems, or those taking medications that blunt the heart rate response to exercise. Side effects may include chest discomfort, flushing, and dyspnea, which are typically brief and reversible.
