Myocardial blush grade (MBG) is an angiographic assessment of tissue-level perfusion that evaluates contrast opacification and clearance from the myocardium during coronary angiography. The grading system ranges from 0 (no blush) to 3 (normal blush with prompt clearance). Grade 0 indicates no contrast density in the myocardium; Grade 1 shows minimal contrast density; Grade 2 demonstrates moderate density that clears slowly; and Grade 3 shows normal density with prompt clearance within three cardiac cycles. MBG provides important prognostic information beyond epicardial flow assessment, as it reflects microvascular integrity and predicts clinical outcomes following percutaneous coronary intervention. Poor myocardial blush despite restored epicardial flow suggests microvascular dysfunction.
