Thrombus burden refers to the extent and distribution of blood clot within the vascular system, particularly relevant in acute arterial or venous occlusions. Assessment includes clot length, cross-sectional area involvement, and downstream propagation patterns. High thrombus burden typically indicates extensive clot formation that may require mechanical thrombectomy devices rather than pharmacologic thrombolysis alone. The concept is particularly important in pulmonary embolism management, where massive thrombus burden may indicate need for embolectomy, and in acute limb ischemia where extensive clot may necessitate surgical intervention. Imaging techniques such as CT angiography or conventional angiography help quantify thrombus burden for treatment planning.
