Aortic arch debranching is a hybrid surgical technique that involves surgically rerouting the brachiocephalic vessels (innominate, left carotid, and left subclavian arteries) to create a suitable proximal landing zone for thoracic endovascular aortic repair (TEVAR). This approach allows treatment of aortic pathology extending into the arch without requiring complete arch replacement. The procedure typically involves extra-anatomic bypasses from the ascending aorta or innominate artery to the arch vessels, followed by covered stent placement across the arch. This technique has expanded the applicability of endovascular methods to more complex aortic arch pathology.
