Full Definition

The foregut hypothesis is a proposed mechanism to explain the rapid improvement or resolution of type 2 diabetes following certain bariatric procedures, particularly gastric bypass surgery. This theory suggests that bypassing the proximal small intestine (duodenum and proximal jejunum) prevents the release of unknown diabetogenic factors or anti-incretin hormones that contribute to insulin resistance. By excluding the foregut from nutrient contact, these putative harmful signals are eliminated, leading to improved glucose homeostasis independent of weight loss. The foregut hypothesis is one of several theories, along with the hindgut hypothesis and caloric restriction, proposed to explain the metabolic benefits of bariatric surgery that occur before significant weight loss.
