Program of Assertive Community Treatment (PACT) is an evidence-based service delivery model for individuals with severe and persistent mental illness who have difficulty engaging with traditional outpatient services. PACT teams provide comprehensive, round-the-clock services including psychiatric treatment, case management, rehabilitation, and crisis intervention delivered directly in community settings. The multidisciplinary team maintains low client-to-staff ratios and uses an assertive engagement approach with shared caseloads. PACT services are typically reserved for individuals with extensive psychiatric hospitalization history and complex psychosocial needs.
