A tissue expander is a temporary, inflatable silicone device placed under the chest muscle (pectoralis major) during mastectomy to gradually stretch the overlying skin and muscle in preparation for permanent breast reconstruction. The expander contains a fill port that allows for incremental saline injection over several weeks or months, slowly expanding the tissue pocket to accommodate a permanent implant. This two-stage reconstruction approach is commonly used when immediate placement of a permanent implant is not feasible due to radiation therapy plans, inadequate tissue coverage, or patient factors. The tissue expander is typically replaced with a permanent implant in a second surgical procedure.
