Burn coma refers to altered consciousness that can occur in patients with massive burn injuries, typically involving more than 50% TBSA. The mechanism is multifactorial, including cerebral edema from massive fluid resuscitation, metabolic encephalopathy from organ dysfunction, systemic inflammatory response, and potential carbon monoxide or cyanide poisoning in inhalation injuries. The condition carries a grave prognosis and requires intensive monitoring of intracranial pressure, metabolic parameters, and aggressive supportive care. Recovery, if it occurs, may be complicated by significant neurological deficits.
