Dermis preservation is a critical concept in modern burn surgery that involves selective excision techniques designed to remove necrotic tissue while retaining as much viable dermal structure as possible. This approach is particularly important in managing deep partial-thickness burns where some dermal elements remain viable. Preserved dermis provides a better foundation for healing, whether through spontaneous epithelialization or skin grafting, and typically results in superior functional and aesthetic outcomes compared to full-thickness excision to fascia. Techniques include tangential excision, enzymatic debridement, and laser debridement, all aimed at achieving this goal.
