High-grade squamous intraepithelial lesion represents a cytologic diagnosis under the Bethesda System encompassing moderate dysplasia (CIN 2), severe dysplasia (CIN 3), and carcinoma in situ. HSIL indicates significant cellular abnormalities with substantial risk of progression to invasive cervical cancer if untreated. This diagnosis requires immediate colposcopic evaluation and typically necessitates treatment through procedures such as LEEP, cold knife conization, or cryotherapy. HSIL findings are strongly associated with high-risk HPV infections and require close follow-up even after treatment.
