Device pocket erosion is a serious complication where the skin and subcutaneous tissue overlying a cardiac device gradually breaks down, potentially exposing the device hardware. Risk factors include thin patients, superficial implantation, prior radiation, steroid use, and poor wound healing. Erosion may present as skin thinning, discoloration, ulceration, or actual device exposure. The condition often requires complete device and lead removal due to high infection risk, followed by temporary pacing if needed and eventual contralateral reimplantation after tissue healing. Prevention focuses on adequate tissue coverage, proper implantation depth, and careful patient selection for device type and placement location.
