fractionated electrogram

Complex, multicomponent electrical signal indicating abnormal conduction typically targeted during substrate ablation.

Full Definition

A complex electrical signal recorded from cardiac tissue characterized by multiple deflections, prolonged duration, and low amplitude components. Fractionated electrograms indicate areas of slow, discontinuous conduction often associated with scar tissue or fibrosis. These signals are critical markers for substrate-based ablation strategies, particularly in ventricular tachycardia ablation and persistent atrial fibrillation procedures. Classification systems exist for quantifying fractionation degree, and modern mapping systems can automatically detect and display these signals. Documentation should specify fractionation characteristics, timing relative to surface QRS, and anatomic distribution.
